Employee Drug & Alcohol History

Per DOT regulations, all employers having employees performing DOT safety-sensitive jobs (driving 26K pound vehicle requiring CDL license) must have on file in employment a DOT drug/alcohol history form on file from the date of their employment. This form link is listed below and needs to be completed by the employer and employee as employer must keep on file readily able to make available for any DOT audit purposes.
